Service of Biological immunology

The main mission of the Immunology Laboratory at Hôpital Cochin is to develop and perform explorations of the normal and pathological immune system enabling:
- Biological diagnosis of autoimmune diseases
- Biological diagnosis of monoclonal gammapathies and cryoglobulins
- Exploration of anti-infectious and vaccine immunity
- Exploration of allergic states
Biological diagnosis of autoimmune diseases:
- The department has been developing a highly specialized immunological diagnostics activity for over 40 years, and performs nearly 90 different autoantibody specificities.
- The autoimmunity activity represented 176,553 acts in 2018. This very high activity is linked to the presence on the Cochin site of a large number of prescribing departments: internal medicine, rheumatology, dermatology, diabetology, endocrinology, hepato-gastroenterology, maternity, in particular.
- The department is affiliated with the DHU "Autoimmunity and hormones" and the DHU "Risks and pregnancies".
- The service also responds urgently to certain pathological situations (severe systemic lupus in relapse, vasculitis associated with acute renal failure) requiring rapid biological confirmation (ANCA, GBM, Anti-native DNA)
Biological diagnosis of monoclonal gammapathies and cryoglobulinemia:
- The activity of this sector represents 69,347 acts
- The laboratory researches and types monoclonal gammapathies and cryoglobulins for hospitalized patients notably in hematology, rheumatology, internal medicine, hepatology.
Infectious immunity, vaccinology:
- Since 2010, the department has performed quantiferon assays for the diagnosis of latent tuberculosis infection.
- The department also explores vaccine immunity and performs diphtheria, tetanus, haemophilus serologies by Elisa.
- The immunology laboratory is the only one in France to offer the opsonophagocytosis technique for pneumococcal serologies, as well as Elisa according to the WHO protocol. This activity is a referral activity for the AP-HP and beyond at national level. The laboratory's expertise enables it to be involved in all PHRCs evaluating the immunogenicity of pneumococcal vaccines.
- The team is developing expertise in the analysis of vaccine responses for candidate vaccines against other pathogens: shingles, malaria, Ebola,....
Allergology
- The department carries out total and specific IgE assays, in particular for monitoring cystic fibrosis patients, an activity that will grow with monitoring to be centralized at Cochin.
